Epuris contains isotretinoin 40mg, also known as 13 cis retinoic acid, a systemic retinoid used for the treatment of severe forms of acne. In Canada, Epuris is specifically indicated for severe nodular and/or inflammatory acne, acne conglobata, and recalcitrant acne that has not responded adequately to conventional first line treatments.

Isotretinoin targets several of the biological processes responsible for severe acne. One of its primary effects is a substantial reduction in sebaceous gland activity and sebum production. Excessive sebum contributes to clogged pores, inflammation, and the follicular environment associated with acne development. By suppressing sebaceous gland activity, isotretinoin addresses one of the major underlying drivers of persistent acne.

Isotretinoin also helps normalize follicular keratinization and skin cell turnover, reducing the accumulation of cells inside pores that contributes to blackheads, whiteheads, and inflammatory lesions. Its combined effects on sebum production, follicular blockage, and inflammation explain why systemic isotretinoin can be effective in severe acne that has failed to respond to topical treatments or oral antibiotics.

Epuris is a specific Canadian isotretinoin formulation and is available in 10mg, 20mg, 30mg, and 40mg capsules. The current Canadian product monograph specifically states that Epuris should not be automatically substituted with other marketed isotretinoin formulations.
