Citalopram is a selective serotonin reuptake inhibitor (SSRI) antidepressant used primarily for the treatment of major depressive disorder (MDD). Citalopram 20mg tablets contain citalopram, supplied as citalopram hydrobromide, and are classified as prescription medication in Canada.
Citalopram works by selectively inhibiting the reuptake of serotonin (5 hydroxytryptamine or 5 HT) at nerve terminals in the central nervous system. By reducing serotonin reuptake, the medication increases serotonin availability within the synaptic space, enhancing serotonergic neurotransmission in areas of the brain involved in mood and emotional regulation.
Unlike some older antidepressant medications, citalopram has relatively limited direct activity at many other neurotransmitter receptors. Its pharmacological effects are therefore primarily associated with its action on the serotonin transporter (SERT) and subsequent enhancement of serotonergic signaling.
Citalopram is indicated for the treatment of major depressive disorder in adults. Antidepressant effects generally develop progressively rather than immediately, and continued treatment may be required after symptoms improve to help maintain therapeutic response.

Citalopram has several important safety considerations. Antidepressants can increase the risk of suicidal thoughts or behaviors in children, adolescents, and young adults, particularly during the early stages of treatment or following changes in therapy. Patients should be appropriately monitored for worsening depression, unusual changes in behavior, or emergence of suicidal thoughts.
Citalopram can also cause dose dependent QT interval prolongation, which can increase the risk of potentially serious abnormal heart rhythms in susceptible individuals. Current prescribing information therefore recommends against doses above 40mg per day and identifies additional precautions for patients with certain cardiac conditions, electrolyte abnormalities, liver impairment, or medications that also prolong the QT interval.
Other potential adverse effects include nausea, dry mouth, sweating, fatigue, sleep disturbances and sexual dysfunction. Like other serotonergic medications, citalopram can also contribute to serotonin syndrome when combined with certain other medications that increase serotonin activity.
Citalopram should generally not be discontinued abruptly after regular use. Sudden discontinuation can produce symptoms such as dizziness, irritability, sensory disturbances, anxiety, headache, and sleep changes. Prescribing information recommends gradual dose reduction when treatment is discontinued whenever possible.
Citalopram is a prescription medication and should be used according to the directions of a qualified healthcare professional.
